Request application field does not populate with value (2012)

SYMPTOMS:
When creating a request, populating the drop down list for application results in a network error. 

*ERROR/WARNING MESSAGES:
Network Error (tcp_error)
A communication error occured

*REASON:
The URL for requesting this field is particularly long. There was a proxy truncating the URL resulting in a wrong request to the web server.

*RESOLUTION:
Configure proxy and other network components to allow long URLs (e.g. > 1000 characters).
