2012 SP1 - Workflow Attach a Survey to Request

*QUESTION:
Is there a way to have a Survey attached to a request that is sent during a workflow and have the results show up in the 'survey results' on the Actions menu of the request?
 


*ANSWER:
If you tag the survey to the entity when creating the survey, it appears in the ‘entity-based surveys’ section. If you tag the question to the entity, it would show in the ‘entity-based survey questions’ section (as an example request entity, this screen would be under Actions menu > View > Survey ratings)
