This Lessons Learned Template(in MS/Excel format) can be downloaded, modified, and used as needed.

At each stage gate in a project, and during project closure, it is important to capture lessons learned. Some lessons will require an immediate process change, and others will have potential impacts on future projects. The template allows for the capture of lessons learned, and categorizing them for easy lookup when undertaking future projects. For this reason, it is a best practice to include a lessons learned review, either during the initiation phase of each project, or at a regular monthly peer review by project managers.
